JPR served as a lead project partner in the development of Milton Adult Day Services’ Care Connections Center at Milton Village, providing comprehensive site design, civil engineering, survey, and site permitting services. This project marked the fourth successful collaboration between JPR, and a larger design team, on behalf of the Center for Hospice Care. From the initial concept to final grading and landscaping, JPR worked closely with the client to shape a physical environment that brings dignity, inclusion, and community to the forefront of adult day care.
JPR’s design of the site’s exterior spaces focused on creating therapeutic and inclusive environments. Key features include raised garden beds, a putting green, a perimeter walking path, and a secluded gazebo — all carefully planned to encourage social interaction and outdoor engagement. Grading was optimized for accessibility, and thoughtful landscaping provides privacy and a calming buffer from nearby infrastructure, ensuring the site remains peaceful and inviting.
Milton Adult Day Services is a groundbreaking program modeled after the internationally acclaimed Dutch “Dementia Village” concept — the first of its kind in the region. The center is designed not only to meet the health and social needs of participants during the day, but to create an immersive, small-town experience that promotes a sense of normalcy, purpose, and joy. Features such as a town square, movie theatre, Irish pub, barbershop, and a garage for hands-on activities contribute to a vibrant, familiar atmosphere.
What truly sets Milton apart is its inclusive philosophy. Caregivers are welcomed into the same shared experiences as their loved ones, offering moments of connection and mutual support. Since opening, the center has become a vital lifeline — helping reduce social isolation, supporting caregivers, and empowering older adults and individuals with cognitive or physical challenges to thrive while remaining in their homes and communities.
By placing community, compassion, and design excellence at the center of this effort, JPR helped bring to life a vision that is reshaping the future of dementia care in Northern Indiana.
